When you come for your appointment please remember to obtain and bring the following:
- Referral letter from GP, family physician, hospital or other doctor
- A GP referral is valid for 12 months. If it has been longer than this, please obtain an updated referral.
- Medicare card, DVA card, pension card
- Have your Private Hospital Insurance information with you
- Reports, X-rays, MRIs, CT scans etc. and any other relevant information
- Please present 15 minutes early in order to fill in the required paperwork.
- Whilst all attempts are made to run to time, there may be circumstances where Dr Lieu may run late due to patient or other factors. We ask for your understanding if this occurs.
What to Wear
- Please dress so that the body part you are being treated for can be easily examined.
